- Aadhaar Card or Enrollment Acknowledgement Slip for each family member aged 5 years and above (Mandatory).
- Residence Proof for the eligible person: Passport, Residence Certificate, Voter ID, Driving License, or Aadhaar Card issued a minimum of 5 years ago in Goa. The attached residence proof should display the issue date.
- Ration Card to establish family details.
- If a ration card is unavailable, include the following documents:
- a) For spouse – Marriage Certificate, Birth Certificate of Child, Passport, Voter ID (with spouse’s name mentioned).
- b) For a dependent child – Birth/School leaving certificate.
- c) For dependent parents – Declaration confirming that Parent(s)/In-Laws are not employed.

- Begin by registering on the official website and subsequently logging in.
- Provide the necessary details for the eligible individual or head of the family.
- Include information about all family members.
- Submit proof of residence, ensuring that it covers a minimum of 5 years. Remember, all attached documents must be self-attested for consideration.
- If seeking a registration fee concession, furnish Caste/Income details.
- Attach the Aadhaar card of each family member. In the case of minors without an Aadhaar card, include their birth certificate.
- Registration fees are as follows:
